EACH are excited to be working with our adult colleagues across Norfolk and Waveney to bring children’s services together to develop our transition into adult hospices.
We’ll be holding monthly sessions to offer young people and their families an opportunity to meet our adult colleagues at Tapping House, Priscilla Bacon Lodge and St Elizabeths Hospice – Zest. They may be involved in your care when transitioning from EACH to adult services, and you will be supported by the familiar faces of our EACH staff during this time.
These sessions will be aimed at young people and their families approaching transition to adult services (aged fourteen plus), as well as young adults who have already transitioned to adult services.
As an introduction to these sessions, we are holding this Transition Information Morning at The Nook on Monday 29th September. This will include a brief talk from our adult hospice colleagues on the services they offer, a chance to ask staff questions, gain information on future events, meet families in similar situations and have an informal chat.
Please note this is a self-care session, so you will be responsible for your young person and any other family members attending.
