We are holding another information coffee morning around transition at Milton hospice on Monday 29th June. This is aimed at parents and carers of young people approaching transition – age 13 onwards.
From 13 years, you might start to hear more about transition. The professionals who support your care start to look, prepare and plan what moving forward from child to adult services will look like. EACH can help with this process. It may feel early, but transition should be a gradual process to allow you and your family to adapt to changes, step-by-step.
This is an informal event with the opportunity to ask questions, gain information, meet families in similar situations, and just have a chat!
This session, we also welcome a parent whose young person has transitioned into adult services, who has kindly agreed to talk about their experiences.
Please note this is not a public event, it is only open to parents and carers of young people approaching transition aged 13+ who use EACH services.
