Family media permission consent forms

Our events are filled with joy, laughter, and unforgettable experiences worth preserving. However, we respect your privacy above all else.

We understand that not everyone wants to be in the spotlight, and that's why we prioritise your consent. We'll never share a photo or video without your explicit permission.

To avoid you and your family members having to complete a media permission form every time we hold an event or when we send you new photographs, we only asking for your permission once and until you notify us otherwise.

If you are happy to provide one-off ongoing consent, please complete the online form applicable to you.

To conform with Data Protection Regulations, we need to ask for a separate form for each individual adult and have a form for young people aged 13 to 17 who have the capacity to consent.

Please also be assured:

• You may withdraw your consent at any time by emailing families@each.org.uk or calling 01223 800800 option 5).
• We will always aim to send you copies of the photos we use.
• We will send you an annual reminder.
